PACT Membership Form
We would love to have all members of the primary care team join the Primary care Academic CollaboraTive (PACT), regardless of your previous clinical or research experience. The only membership requirement is an interest in improving primary care through collaborative research and quality improvement projects!
Your information will only be used for the purposes of contacting you regarding opportunities to collaborate on PACT projects and news about PACT’s activities, including via our monthly newsletter. Your details will not be shared with third parties* without your expressed consent. You can end your PACT membership at any time by contacting the PACT secretary via email (gppact@gmail.com).
We use Mailchimp to deliver our newsletter, and we will share your information with Mailchimp for the purpose of managing this mailing list. Mailchimp also automatically collects your IP address (your computer's unique identifier). Learn more about Mailchimp's privacy practices here. By subscribing to this list, you agree that we may process your information in accordance with these terms.
